Application Instructions - Thunder Bay Catholic

BEFORE YOU BEGIN

 

Thunder Bay Catholic District School Board will only accept complete applications using our secure online application system.  Mailed, emailed, or faxed applications are not accepted. Incomplete or unpaid applications will not be processed.

 

Before you begin the online application process, please make sure you have collected all the necessary information and documents.  You will need the applicant’s complete information and at least one parent’s complete information. This includes names, birthdates, addressed etc.  Required and recommended documents (see below) must be submitted as PDF or JPEG files only and less that 5 MB each. If any document is larger than 5MB, it has been scanned too high a resolution and must be rescanned at a lower resolution before beginning the qapplication process.

 

The application fees $250.00 CAD and is nonrefundable.  Please have a major credit card ready for the final stage of the online application system.

 

REQUIRED DOCUMENTS

 

In order to submit an application, the three documents listed below are required.  If you do not upload these documents into the online application system, you cannot complete the application process.  If you do not submit a complete application within seven (7) days, the partial application will be automatically deleted.  The Application must include the following:

 

  1. Participation Agreement.  The parent(s) and student must sign our agreement.  Before starting the application process, please download Participation Agreement , have it signed in ink, and scanned as a PDF or JPEG file only.
  2. Transcripts or report cards.  You must have a record of grades for the last two years, translated into English (if possible), they must be in  PDF or JPEG files only, and less than 5MB in size. You will only be able to upload one document, so please ensure all pages of the records of grades are scanned as a single document.
  3. Copy of Passport/Birth Certificate
  4. Recommendation letter to TBCDSB for evaluation.  Please have a teacher, school counsellor or Principal write a letter of recommendation for the applicant.  This letter should identify the student and their strong points.

 

APPLICATION PROCESS

  1. Students submit the application form, application fee, copy of passport/birth certificate, recent 2 year official transcripts or report cards.    All documents must be in PD format. Recommendation letter to TBCDSB for evaluation, either by mail or email.

  3. Successful applicants receive Conditional Letter of Acceptance (CLA). Student pays tuition fee and submits custodianship documents. 
  4. Successful applicants receive Official Letter of Acceptance (OLA) for use when applying for a Study Permit and Visa to Canada. 
  5. Students report to the TBCDSB with passport, study permit and other missing documents.  Medical insurance coverage shall be activated upon reporting.

May 31st for the September entry

December 1st for the February entry



 DOCUMENT CHECKLIST

 

The following documents are mandatory at time of submitting the application:

 

  1. Participation Agreement, signed and scanned as a PDF or JPEG.
  2. Copy of passport (photo page) or Birth Certificate.
  3. Copy of most recent two-year transcripts or school report cards (certified or notarized).
  4. Copy of Completed, Signed and Dated letter of Recommendation.

 

The following documents must be submitted to TBCDSB before July 1st for September Entry and December 1 for February Entry or during the reporting Stage:

 

  1. Copy of Custodian Declaration Form  (Parents)(Notarized) (see link to download)
  2. Copy of Custodian Declaration Form (Custodian) (Notarized)
  3. Proof of Custodian’s Status in Canada (Copy of Passport, Citizenship or Permanent resident Card)

 

At time of reporting:

 

  1. Copy of Passport
  2. Copy of Valid Study Permit
  3. Copy of Immunization Records
