ELEMENTARY (K - GRADE 7) ACADEMIC PROGRAM (CREDIT-BASED)

 

Required documents

1. A recent photo of the applicant taken in the last 12 months.
2. Picture page of the applicant's Passport or Personal Identification.
3. Certified or notarized School Records/Report Cards translated into English from the current and previous year of study. 
4. Picture page AND the signature page of the signing parent's Passport or Personal Identification.
5. Signed Declaration of Eligibility. (you can download this form once you have logged-in)
6. Signed and Notarized Declaration to Accompany Minor child in Elementary School. (you can download this form once you have logged-in)
7. 1 recommendation letter from current teacher or principal.
8. Immunization (Vaccination) Record. (preferably translated into English and notarized)
9. Vancouver Coastal Health - School Form. (you can download this form once you have logged-in)
10. $200CAD application fee (non-refundable).

Additional documents (optional)

1. Letter of Study Intent.
2. Awards and/or certificates in extra extracurricular activities.
3. Student Current Study Permit. (if applicant is currently studying inside Canada)

 DOCUMENTS UPLOAD requirement

• Documents must be in PDF files only, and less than 5MB in size.
• You will only able to upload one file per document, so please ensure all pages of the same document are as a single document. For example, scan all pages of your report cards under one file.

 PHOTO SPECIFICATION

• The photo must show the full front view of the head, with the face in the middle of the photograph, and include the top of the shoulders.
