Required Documents

Before you begin, please collect the following information and documents to be uploaded:

1. Transcripts or report cards for the past two years, translated into English;
2. Passport photo page, including identifying data;
3. Immunization record, translated into English;
4. Mandatory Student Participation Agreement, signed by the parent(s) and student (found in Section 8 of the application);
5. Parent(s) contact information including email address;
6. Education program start and end date;
7. Medical information.

Application Fee

Payment information for the application fee is found in Section 10 of the application. Applications will not be processed without payment.

 

Notes

• Documents must be uploaded as PDF or JPG files no larger than 5 MB each.
• Mailed, emailed, faxed or hand-delivered applications are not accepted.
• Incomplete applications and/or applications with illegible documents will not be processed.
• Applications will be processed upon payment of the application fee.
• Applications automatically expire after 30 days. 
• The application fee is non-refundable
